Your K-State Student Union accepts passport applications on behalf of the US Department of State. US citizens planning international travel may apply for their passports in the Union director’s office on the second floor of the Union. No appointment necessary!
Applications forms are available at the links to the right, on the web at travel.state.gov, or in the director’s office.
Steps
- Complete the application and bring to the director’s office.
- We submit your application to the State Department
- You will receive your passport in 6 weeks in the mail.
What to bring with you
- Unsigned Passport Application Form
- Proof of US Citizenship (one of the following)
- Certified US birth certificate
- Previously issued, undamaged US passport
- Naturalization certificate
- Certificate of citizenship
- Proof of Identity (one of the following)
- Valid driver’s license
- Current government ID
- Military ID
- 1 x 1.25 in. photo (can be taken in the director’s office)
- Applicable fees (see below)
- PHOTOS:
- Your passport photo can be taken in the director’s office.
- For children under age 16, the child and both parents or legal guardian(s) must appear in person and present the following:
- Evidence of the child’s U.S. citizenship
- Evidence of the child’s relationship to parents/guardian(s) AND
- Parental/guardian identification
- If only one parent appears, you must also submit one of the following:
- Second parent’s notarized written statement consenting to issuance for the child with photo copy, front and back, of non-applying parent’s ID
- Primary evidence of sole authority to apply
- A written statement (made under penalty of perjury) explaining the second parent’s unavailability

The U.S. Passport card can be used to enter the United States from Canada, Mexico, the Caribbean, and Bermuda at land border crossings or sea ports-of-entry and is more convenient and less expensive than a passport book.
