What do I do if the incident is an Non-Emergency?
Answer
If the incident requires any non-emergency medical care (including urgent care) the employee should take the following steps:
1. For injuries requiring medical attention but not an emergency room visit (e.g., sprains, cuts, minor burns) the employee may seek care at an urgent care center.
2. Before or after visiting urgent care, the injury must be reported to the Union's Workers' Compensation Provider, SFM, by:
- Calling the Nurse Triage Line at (855)-675-3501 (choose option 1), following the care instructions provided.
Or
- Report the injury online. Policy number: 128341.201
You will need the following documents when you seek treatment:
- Insurance Identification Card
- Work Ability/Return to Work Form - Must be filled out by the attending physician and returned to Union HR upon completion of your appointment.
